I have Piles?(quite disgusting)?

I have been having trouble for a while with piles, well i thought it was an anal fissure, but i went to the doctors today and discovered its wasn't. It was piles, my doctor gave me a laxitive, that i am going to start using tomorrow and told me to drink lots of water. However i got home tonight and went to the toilet, i again strained with a bowel movement, looked into the toilet, and there was lots of blood. That has never happend before only blood on the toilet tissue. I am getting pretty scared, as i have never had anything like this before. Does this amount of bleeding mean it is healing or not :O Shall i contact my doctor, tell her it has got worse? Or should i do what she told me and if the laxitive doesnt work go back? paranoid here !! Please help!
